A lookup path you can follow
Use the directory links in the source box below. Keep the names beside you while you search.
Get the exact names
Get the health professional’s licence number and full name, along with the sending pharmacy’s legal name.
Find the right medical board
Use the Federation of State Medical Boards directory to reach your state’s medical board, the body that keeps doctors’ licence records.
Read the licence record
Check the licence’s status and any public disciplinary actions, meaning formal steps the board took against the doctor.
Check the pharmacy separately
Use the Food and Drug Administration (FDA) state pharmacy lookup to find the pharmacy in your own state’s board records.
What the record does—and does not—tell you
A public licence record can reveal a doctor’s licence status and any action taken by the board.
- A clear record is not a rating of care quality.
- Some states use separate licence boards for different types of doctors.
- The pharmacy’s licence record is separate from the doctor’s.
- A pharmacy listing confirms that it has a licence. That alone does not prove that each medicine is handled and supplied properly.
- Ask separately whether the exact medicine may be supplied and how its quality is checked.
