A set list of questions has limits
A fixed question-and-answer form cannot be the sole basis for prescribing. Medical-board guidance rejected that approach at the September 26, 2026 check.
The Federation of State Medical Boards links bodies that oversee doctors and provides this guidance.
- A health form can be part of care when the health professional can ask you more questions and gather the details needed.
- The board guidance is not a nationwide law. State rules differed at the September 26, 2026 check.
- Ask how the service handles an answer that does not fit a box.
Make space for your questions
Patients also need a way to ask more questions after the first exchange. The board group said this in its May 2026 guidance.
- Write down the question you most want answered.
- Bring a list of medicines, supplements and past reactions.
- Ask who replies after the visit and how to reach that person.
A screen cannot do every exam
When an assessment needs a test or an exam in person, medical-board guidance calls for it to be arranged.
- Ask what the test would answer.
- Ask who orders it, explains it and bills for it.
- Note what is still undecided at the end of the conversation.
